Privacy Policy Effective Date: 6 April 2013
Personal Information Collection
FrontierMEDEX, Inc. (herein “FrontierMEDEX”) collects Personal Information about you when you provide such information to us to purchase a product or service, become a member of one of our online resources or when this information is given to us by affiliated entities or third parties. "Personal Information" is information that would allow someone to identify or contact you, including, for example, your full name, address, telephone number, or email address. In addition, depending upon the service that we provide, we may collect information about your travel plans, medical information to create an emergency medical record or when you call in to our Emergency Response Center to open a case. Personal Information does not include aggregated or bundled information that, by itself, does not permit the identification of individual persons (such as statistics on the number of persons visiting the FrontierMEDEX website each month).
You provide certain Personal Information to us when you: (a) order products and services from FrontierMEDEX websites (including frontiermedex.com, medexglobal.com, cmi-insurance.com and medexassist.com), (b) submit forms through the Member Center website (c) send other information to us through the "Feedback" interface, or (d) take an online survey. Your employer, or an affiliated entity, may provide information about you to us to enable us to provide you and your employer services, such as travel related services.
You may also wish to subscribe to one of our newsletters, receive a white paper or attend a Webinar for which you must opt in or enroll. At that point we ask for contact information such as name and email address. Users who no longer wish to receive a future newsletter, notice of a white paper or a future notification of a Webinar can unsubscribe when they receive it by clicking on a link at the bottom of the newsletter or notification. The user will then be given an opportunity to opt in or out of future newsletters or notifications and opt in or out of further periodic FrontierMEDEX communications.
Email forwarding functionality is provided in many of our communications. Using this functionality will forward the communication privately. However, if you use the forwarding methods, like those provided by your email server, they may transfer personally identifiable information. These email messages may contain "clear GIFs" (Graphics Interchange Format, a type of picture file used on the internet).
In the event of a transfer of ownership, a sale of assets, or a bankruptcy of FrontierMEDEX, your personal information, gathered from purchasing or making reservations for products or services or electing to receive communications (such as emails), may be passed on to any resulting third-party entity. We may also disclose specific information when we determine that such disclosure is necessary to comply with the law or with any legal obligation to which we may be subject.

Use of Personal Information
FrontierMEDEX will use your Personal Information to provide you with services that you purchase, or services that are purchased by your employer or affiliate. FrontierMEDEX uses the information that you provide to respond to requests and later potential communications regarding the product or services you have purchased. These services are designed to provide tools for risk management, or medical and security assistance. FrontierMEDEX may use your Personal Information to perform analytics and to improve our products, websites, and advertising. Your Personal Information may be transferred to affiliated entities that are performing services for FrontierMEDEX or acting as its agent. We will require that any such entities or persons providing services on behalf of FrontierMEDEX agree to protect the privacy of your Personal Information and use it only for the purposes for which the information is transferred. FrontierMEDEX will not use or disclose your Personal Information, except as described in this policy, unless you give us your permission to use or disclose it for other purposes. FrontierMEDEX will not sell or rent your personally identifiable information to third parties. FrontierMEDEX will take reasonable steps to ensure that data is reliable and relevant for its intended use, accurate, complete, and current. FrontierMEDEX will retain your personal information as long as we need to perform a use listed in this notice.

Dispute Resolution
If you have a complaint about any aspect of our website privacy policies please bring it to our attention by addressing your concerns to privacy@frontiermedex.com Please prefix the subject of your email or mark your envelope 'confidential'. We endeavour to resolve all complaints as soon as they are brought to our attention, but if this is not possible, we will acknowledge receipt of your complaint in five working days and advise you how we intend to resolve the issue you have raised and how long this is likely to take. Complaints and disputes will be resolved expeditiously.
If you are not satisfied with our handling of your complaint, you may contact eTrust at http://www.etrust.org/complaint/raise_privacy_complaint.php. FrontierMEDEX commits to cooperate with eTrust in the investigation and resolution of complaints brought under the Safe Harbor. In addition, FrontierMEDEX agrees to comply with any advice given by eTrust with regard to specific action needed to comply with the Safe Harbor Principles, including remedial or compensatory measures for the benefit of individuals affected by any non-compliance with the principles.

Log Files
We use IP addresses in log files to analyze trends, administer the site, track a user's movements, and gather broad demographic information for aggregate use. Log files do not enable us to access personal identifying information, such as email addresses.

US-EU and Swiss Safe Harbor Membership
In accordance with our commitment to protect personal privacy, FrontierMEDEX, Inc, adheres to the principles of the Safe Harbor Framework as developed by the U.S. Department of Commerce in consultation with the European Commission and the Swiss Safe Harbor Framework. The seven principles and fifteen Frequently Asked Questions (FAQs) referred to in this policy constitute Safe Harbor privacy framework. These principles and FAQs may be found at http://www.export.gov/safeharbor. The Safe Harbor framework provides guidance for the U.S. organizations on how to provide "adequate protection" for personal data from Europe and Switzerland as required by the European Union's and the Swiss’ Directive on Data Protection. FrontierMEDEX undergoes an annual privacy review and verification process as part of its adherence to the Safe Harbor principles.

Disclosure Policy and Disclaimer
Personal information may be disclosed to judicial or other government agencies subject to warrants, subpoenas, or other governmental orders.
FrontierMEDEX respects and protects the privacy of its clients. Personal details, including email address and telephone numbers are treated in confidence. These will not be disclosed to any third-party, except in connection with the provision of our services. All personal records are used for the sole purpose of providing services. FrontierMEDEX complies generally with international laws on privacy protection in relation to the storage and processing of personal data.
FrontierMEDEX facilitates the transfer of information, including, but not limited to, medical records, between its member/client/customer (wherever they are in the world), their employer or family members, their doctor or medical specialist, and local authorities and physicians in situ. Any member/client information stored temporarily by FrontierMEDEX is held in a secure environment and is not listed or associated with the name of the member/client (or a member/client who is a medical patient), thus retaining confidentiality at all times and in all respects. Furthermore, no information is released to anyone without the express permission of the member/client (or an authorized representative of same) as well as appropriate warranties or confidentiality/non-disclosure agreements.
Notwithstanding the above stated FrontierMEDEX policies concerning the disclosure of information, FrontierMEDEX must disclose information of members/clients if compelled to do so by judicial order or valid governmental order or decree. However, wherever possible, FrontierMEDEX shall give prior notice to the member/client of the order compelling disclosure and shall cooperate with the member/client in any efforts to prevent disclosure or to limit the disclosure.
